Honoring Choices’ five-hour Advance Care Planning Navigator Training will provide you with an overview of advance care planning, and the knowledge and skills necessary to guide others through advance care planning conversations with the Honoring Choices ACP Conversation Guide. You will also learn about the requirements for completing a Minnesota health care directive, the differences between a health care directive and POLST form, and trusted ACP resources. 5.0 CEUs will be awarded upon completion of the class and its prerequisites.
