Advance Care Planning
Advance Care Planning (ACP) is a process which helps you think about, talk about, and write down your choices for future health care decisions. While it's not an easy topic to consider, it is important for every adult to have a Health Care Directive - a written plan for loved ones and health care providers to follow - so that your wishes are known if a time comes when you cannot speak for yourself.
Honoring Choices Minnesota is focused on helping every adult Minnesotan understand what Advance Care Planning is, and working with health care providers to make sure they offer assistance to all patients, and will honor your choices.

Honoring Choices works with health care providers and caretakers to build strong ACP programs and to increase personal and professional skills. We offer training throughout the year and we can help you design and begin an ACP program which will work in your environment.

Advance Care Planning is a process of looking at what might happen in the future - if you or someone you love is ever in a position where communication just isn't possible. It could be the result of a long illness or a sudden and serious accident. When someone cannot communicate, their loved ones and their health care team want to provide the best care possible. ACP lets the patient's voice be included in the decision-making process.
