Health Care Directives

A health care directive, also known as a living will, is a document in which one can express their values and preferences. This document serves as a guide for your health care agent/decision maker, family, and care providers, providing crucial information about the type of care you desire and interventions you wish to avoid. It can communicate the care you desire to receive when circumstances prevent you from expressing your preferences directly.
